When you complete the New Patient Intake Form, we’ll have an excellent understanding of your past and current health situation. The form may seem lengthy at first but most of the questions are “Yes” and “No” answers, so it will go quickly. But, more importantly, the information you provide is crucial for determining the best course of care. Want to make it go even quicker? Check out the "New Patient " tab of the website to download the form that applies to you to fill out ahead of time!
Consultation:
Once you’ve completed your paperwork, you will meet with Dr. Avy and discuss your health concerns for yourself or your child. Here, we will find out if you’re a good candidate for chiropractic care!
Examination and Treatment.
Following the consultation, we'll perform an examination. The purpose of this examination is to locate the cause of your problem so we can develop a plan to help you.

Dr. Avianne ‘Avy’ Waithe received her doctorate from Parker University where her areas of concentration included several soft tissue therapy techniques, pediatrics, nutrition, postural rehabilitation, and spinal rehabilitation. She has devoted her life to helping others live life to the fullest potential by enhancing the quality of her patients lives by combining the very best hands-on-techniques, rehabilitation methods, and nutrition with a highly individualized approach when caring for her patients. Being a Board Certified Chiropractor in the state of New Jersey,she practices functional medicine in nutrition and is an occasional columnist. Dr. Avy has been actively practicing in multidisciplinary settings alongside orthopedic surgeons, primary care physicians, pain management specialists, and naturopathic doctors. She has experience treating athletes, families and babies, while specializing in prenatal, pediatric and family wellness. Furthermore, she is a active member of the American Chiropractic Association, Holistic chamber of commerce and other nutritional and holistic associations. Dr. Avy is excited to be in part of North Bergen, NJ and Jackson Heights, NY communities and looks forward to meeting and serving you, your family, and your friends.
